We utilize an electronic billing software for our practice called
ChiroTouch. This allows you to complete your paperwork online and it submits directly to our office software. There is no need for your to print out to complete. Please click on this link:
Our practice has been compliant with the statute that ALL
healthcare providers by 2015 will have to create and maintain medical
records in an electronic format since June of 2011. You will see that
Chirotouch is not only easy to use but is fully HIPPA compliant and all
information is secure and will not be shared with unauthorized users.
This system helps us work together to maintain safe, secure and accurate
up-to-date medical records for you. From now on, your important health
information will be available to you when you need it, from any computer.
You, as the patient, will be able to view upcoming appointments, receive
appointment reminders, receive text messages and share medical records from
other doctors with Dr. Bell.
Please make sure our office has the most updated insurance information and we will do our best to verify your chiropractic insurance benefits before you arrive for your new patient appointment.
If you do not have access to a computer or do not have an active email
address, you can complete this paperwork in our office (please arrive 10-15 minutes earlier).
